How to submit a claim form


If you are claiming for Medical, Dental, Nursing or Allied Health Reserve Officers please refer to the 'Claims for Health Professionals'. Claims for all other Reserve categories should be submitted to the   relevant   Service Delegate as listed below.

Navy
 
If your employee is a Naval Reservist, the ESPS claim form should be submitted direct to The Navy ESPS Co-ordinator

Army
 
Army has appointed delegates on a regional basis, within most regional Army Personnel Agencies, for ESPS claims relating to service by Army Reservists. If your employee is an Army Reservist, the ESPS claim form should be submitted direct to the nominated ESPS delegate for the geographic area. Special arrangements apply for members working within:
  • Army Headquarters, or
  • Non-Army elements in the Australian Capital Territory

Air Force
 
If your employee is an Air Force Reservist, the ESPS claim form should be submitted to the Commanding Officer of your Reservist’s Air Force Reserve squadron. If the member is not posted to a Reserve squadron, the claim should be submitted to the Reserve squadron closest to the member’s Defence work location (for example, claims relating to an Air Force Reservist who is not posted to a Reserve squadron and is employed in a Defence Materiel Organisation office in Melbourne should be submitted to 21 SQN).
